I'd support this, with a few caveats.
1. It should be limited to facts only, no speculation etc.
2. It should most certainly be closed and require registration
3. It'd need active moderation
4. It's purpose should be purely accident analysis to allow identification of root cause and allow others to learn from this.
This is always an emotive subject, but at the end of the day, if we dont look at why/how mishaps happen, how can we improve "our" safety.
