Also why wasn't the drysuit dumping on ascent - was there a fault with the valve or was is likely something to do with a horizontally trimmed ascent not allowing the dump to work efficiently?

There isn't a difference between ascents rates in T1 and T2 (9m/min to 80%, between 80% and 65% ATA depends on exposure but could be as high as 6m/min, the 65% to switch is 3m/min). However, the importance of getting moving in T2 is greater as a 'slow' ascent rate at T1 level won't have that much of an impact on deco at the 45m/30min 1:1 baseline but at T2 where at 66m it becomes 2:1, any extension in bottom time (which could be read of as a 'slow' ascent) will have a more marked effect on the deco requirement.
Horizontal trim does allow the dump valve to work properly but admittedly once you have air in your feet it is much harder to get rid of it if you are horizontal. However, if you are under control (which Mark clearly wasn't) then it isn't an issue.
