It's a very common misconception, caused by such programmes as Casualty etc, that a Defib starts a heart that has stopped. It doesn't, it actually stops the heart (hence my shouting at the poolside) in the hope that it will restart itself in the correct sequence!
There are only two rythmns that can be shocked and only a Defib can determine these in the field!

And I'm with you Dlegros CAB does seem the way they are going, and what happened to 15 compressions to 2 breaths, 30 to 2 doesn't work singing Nelly The Elephant to maintain correct speed!

Not a problem at the minute though, my trust can't decide what to do yet so we're still ABC and 15 - 2!!!

The 30-2 version is what I have been taught as my friend is on the cutting edge side of the training and that is what she is teaching. I was happy about it as it removes all doubt and makes things super easy for me. No more fannying around, and straight to the compressions.

Thank you for this thread. Having defib operator and any bystanders away from any water whilst operating the machine is not something that I remember learning about but as has been pointed out is very important.

Tried to reply earlier in the only quiet spot at work (about 08h00) but it failed!

I still find Nelly works, you just sing it through twice, or learn the second verse!

That's right folks, it's the bit they never show you on Casualty, Holby Shitty, Trauma, any of those - medical professionals synchronising CPR to Nelly the Elephant!

PV - atrial fibrillation is not a defibrillatable (sp?) rhythm. If the ventricular response is sufficiently fast to compromise cardiac output, then you would consider a lower-energy r-wave synchronised shock (slightly different to just defibrillating) and you require a manual defib with trained operator, plus sedatives as it hurts! Ventricular Fibrillation mandates a shock (360J monophasic, 150J biphasic), as does ventricular tachycardia without a pulse.

Follow the emphasis as you sing and you have 15 compression at 100 per minute.

I got taught his on an ACLS course and have always passed it on since.

Glad to be of service

Whenever I teach this on EFR, I always get incredulous looks - then demonstrate and can see the light dawn across the room.

And yes, I do sing it (to myself) at work occasionally to check my rate, unless I'm facing the defib then I can see my rate on screen.
