Dave,

If you need a referee, that is the list. You may not need to visit - I just sent my stuff in the post with a cheque. If they want to see you, they will let you know.

If it is for an HSE med, I think the options are greater. Probably still not good in your area.

Bit more serious than that - friend has just been diagnosed with diabetes need a certificate to dive
