Of course, it's not only divers that have these events, believe it or not skippers are prone to the occasional "faux pas" (?sp) (cock-up or act of god!)
I went out to the "our daddy" one cold windy February morning in preparation for a fuel run up to poole harbour, as I came alongside the boat in the dinghy, I held on to the side of the boat and reached to disengage the gears on the outboard motor, this movement coincided with a strong gust of wind which got under the bows of the dinghy, and, helped by my weight being towards the stern, promptly spat me out over the transom! I clearly recall seeing the transducer of the echo sounder (and thinking to myself that I should scrape some of the growth off of it soon) on the hull of the boat, then I kicked for the surface, I was a bit alarmed to see the bows of the dinghy having decided that it was going to "ave anuver go! and managed to turn and duck my head as the bastard came down on my shoulder, so off I go for another unscheduled
look at the transducer (Yep, it certainly does need a scrape off) and a rapid climb back in the boat.
oddly, the outboard motor was still running even though the dinghy was capsized and down by the stern, and I, notwithstanding 2 visits to the underside of "our daddy" was only wet from the waist down.
bloody cold on the way up the harbour but it could have been worse.
mike.

Bloke in our club got all the way to the deck of the Thesis then realised he hadn't got any fins on. Went back up the line to see a couple of Inspo divers competing a safety stop. Was too embarrassed to go past without his fins so waited until they had finished until he surfaced.

I did go out on a boat once to dive a fairly popular wreck - it has a permanent shot and is quite shallow. I was the first one down the shot but couldn't understand why it seemed to be a bit loose. I got to the bottom to find the 'shot' was attached to a lobster pot. The proper shot was about 10m away and firmly attached to the wreck.

I've done the snorkel thing, but was fortunately DM'ing a bunch of OWs at Capernwray and was riding shotgun, so I was able to casually do a snorkel-regulator exchange drill at 1m without anydody noticing.... the coughing subsided without me needing to surface....

Unfortunately for my buddies, I generally have my camera in hand, so there is some evidence of their misdemeanours on record....

there are others, but this is the funniest

A couple of years ago on a very full day boat in Sharm we were waiting for a Dutch dive club to kit up and jump in. After about half an hour somebody in our group suggested they might like to get wet sometime soon, to which their leader gave everyone a safety lecture and with 2,000 dives logged he wasn't going to compromise just because we were waiting. He then took a giant stride off the back of the boat without his fins on.

I fogrot about this one....
I was diving with a CD (just to dispel the myth that they are perfect ) about 4 years ago, not long after I had completed my IDC.

We descended to about 15m when the CD tapped me on the shoulder and pointed at his first stage, signalling for me to turn the valve. Logic told me to turn it anticlockwise.

Remember that bit in your course that says 'make sure you open your valve fully'? Well, it seems that at some point, he had only opened it a little bit. Enough to breathe on the surface, and as we descended, it got harder and harder to breathe.

Oh, how I wish I had taken a credit card swipe machine

It's a good job someone taught me 'righty tighty, lefty loosey', otherwise it could have been a bit messy.
