| You could look through the 'I Learned About Diving From That' sub-forum for incidents. However, what I think you are talking about is the BSAC Annual report. Have a search on here & you should find a link.

| There is also the Dive Accident Repository on Yahoo Groups. Not sure if it is still active but the archive should be there.

| The BSAC reports are multi-agency. I think most agencies (including PADI) support and contribute to them. They are UK-centric and I'm not aware of ones outside the UK. Janos
