Things went wrong that perhaps shouldn't have (with the said hindsight) but it takes a big man to post such an open and honest account for others to learn from and serves as a wake up call to everyone at the beginning of this year.

Having just practiced that exact scenario in a quarry with Mark in what was a relatively controlled environment and still managing to let it all run away with us it is nice to know that the training does stick and will work in a very real situation.
