You of course are correct about the waiver, I would normally tell my students that as I show them the form, and discus it with them, but we must still get all students to sign the form to be within standards.
The medical statement is simple to understand, from the PADI medical form,
"Please answer the following questions on your past or present medical history with a YES or NO. If you are not sure, answer YES. If any of these items apply to you, we must request that you consult with a physician prior to participating in scuba diving. Your instructor will supply you with an RSTC Medical Statement and Guidelines for Recreational Scuba Diver’s Physical Examination to take to your physician."
Now it may be your instructor was lacking in social skills and that's why your nose was put out of joint, some of these young instructors are, well young, but as you wrote yes on the medical form, you must go and get a doctors medical, by standards.
