Hi Adrian,
            They just go on an incident report form like BSAC. It gets sent to HQ and they deal with it from there. The DO's keep the forms. I reckon that a "vast" percentage of SSAC's incidents go unreported as their seems to be a great deal of problems, but no DCI/fatality, that get mentioned on the report that would never even get mentioned at a club meeting in SSAC.

'Tis not a good thing in the bigger picture really.

I thought BSAC were supposed to be the governing body and therefore KNOW EVERYTHING.  Perhaps DDRC or similiar need to take over the incident reports, seeing as BSAC don't get all of the info.

The British Hyperbaric Association (BHA) have tried to compile a UK-wide diving incident database before (still are?), but even they have had difficulty getting all cases reported.

There has been collaboration between BSAC and BHA on this issue here, not sure if there still is.

is it me or does most of these incidents concern quite well qualified divers pushing the limits as a diver its very distressing to see reports like this and wonder if more could,nt be done to save lives.I know that when we dive we all take a certain risk just goes to show how fragile human beings are in that great big blue

I dont know about pushing the limits. Lots of repeat dive DCI is just ignorance of risk. The livaboard trip drinking all night and diving all day and not a care in the world. No proper hydration that sort of thing. As far as the experianced diver incidents are concerned I am aware of the details on a couple of them.

The 60m dive with the diver entangled in netting was an air dive carried out by very experianced ex comercial &nbsp;divers.

The diver who went to 60 solo did so with low presure in his tanks at the start of the dive, a decision thaught odd by the other divers on the boat. He was a stranger to the rest of the group so little skills knoladge was known. No body knows why he didnt swap to travel gas at 40m but having read Deep decent I consider panic related C02 black out to be a possabuility.

One of the chaps who convulsed on the 6m stop was known to be experimenting with high CNS and the O2 window and it bit him.

I dont know if these should be considered as pushing the limits. Its more divers choosing to dive outside established safety peramiters and paying the ultimate price for their decision.

This is not a criticisum as I have made the same mistakes my self and just been lucky so far.

i do agree mark!!!! its probably how i worded it??be it taking unessacary risks or pushing the limits its still tragic when a diver (falls) to the sport in fact we have spoke about this subject at stoney today on our surface interval and a few do seem to have been preventable?
