.html form design help required

I am trying to put together a webpage together which has a series of boxes which will be filled in for ordering some prints.

The following are required: 'Name', 'Address', 'Email address' & 'Telephone Number'

The boxes would be 'Reference' (free-text), 'Size' (drop-down) & 'Quantity' (numerical free text).

I would need around 10 boxes to start with but there should be an option to increase the numbers of boxes 10 at a time. No maximum number of boxes is required.

At the bottom of the page, I would like to have a button which says 'Paypal' or 'Cheque'.

There is then a submit button at the bottom which then sends an email to my address with the order details.

Anyone help out

Son, you're going to have to make up your mind about growing up and becoming aircrew. You can't do both.

The aircraft limits are only there in case there is another flight by that particular aircraft. If subsequent flights do not appear likely, there are no limits.

Is it publically viewable?

Designing the form is all in html, but you'll need a bit of code to do the actual emailing, so need to know what scripting is available.

I used to use FormMail - costs a couple of quid but was superb, support was extremely quick.
